i'm trying to figure-out which adjustment's do i need to make in order to insert my hand from the puppet's head, and not throu the body.
do i still attach the head and the body the same way?(with a sleave)
do i need to change the mouth-plate? mouth-plate greep?
do i need to reinforce the entrance on the back of the head?

You shouldn't need to change the mouth plate any.  You would still connect the head to the body in the same way but you do open up the back of the head so you can get into it.  I have in the past opened up the seam in the neck right below the back of the head as the entrance to the puppet.  This seems to work pretty well for a vent type puppet. I really did not reinforce the entrance except to stitch the seam allowance down around the opening.  If you just cut the hole I think it would fray over time.
