Looks to me like a woman turning her kid into a marionette. It's a popular halloween costume idea (spend 5 minutes on Yahoo Answers before Halloween and search for marionettes. It comes up every year) and it wouldn't surprise me that some people try to find cheap alternatives - there are a couple of dress pattern companies that offer marionette costume designs.

I was just wondering as it looks like the stand and jackets and things have been well made. From what I can tell it's a puppet shop in Arglinton (something like that, from the markings on the ambulance)
