Given the character it would be creepier to have no eyes or maybe eyes but no pupils, just the whites. I think you definitely need a mouth. for the construction I would suggest a fleece tube filled with polyfiber fill. I would then cover the tube with nylon from women's hose. This will have the stretch you need, the smooth look of an earth worm, in the color you want.
I know this was a bit sketchy so if you have questions ask away.

Jon has a great idea to spring board off of his idea....what about a smaller size dryer vent tube....the silver/white coloured ones and cover it with a light layer of flat stuffing (used in quilts) to hide the colour and some of the ribbing and then use the panty hose to cover all this. Just another thought.....

no it has not worked aaaarrrggggg back to the drawing board however the head has worked i have used a litter picker to operate the mouth and it seems to work ok.
this is what im thinking with the worm's body if i got some brown fabric and rolled it over and sowed it and then filled it with the little white beans you get in bean bags that should work right!!!!!.

What's the problem you are having? Is it the look of the puppet or the movement?
I know snakes and worms can be challenging especially when you are trying to get the look you want combined with realistic movement.

I think tht stiqman's suggestion of the dryer vent hose is good. That should give you the worm movement.Try and see!
